Referral Criteria
Patients suffering from advanced cancer, Motor Neurone Disease, multiple sclerosis, Huntingdon's chorea or AIDS, may be referred to the Hospice by their General Practitioner or Hospital Consultant.
The catchment area for the hospice includes the City of Chester, the rural Chester area, Ellesmere Port and Deeside.
How can I receive help?
Patients requiring referral to the Hospice should ask their GP or hospital doctor to send a referral to the Hospice. Patients or carers may arrange an informal visit by contacting the Hospice by telephone.
Requests for a Macmillan Nurse can be made via your general practitioner, district nurse, health visitor or hospital consultant.
